1. Data controller

The business is run as a sole proprietorship under the name LOGOPEDA KINGA KRAJS, NIP (tax ID): 6423255638, REGON: 529511490. For data protection matters, write to: kingakrajslogopeda@gmail.com.

2. Scope of this policy

This policy covers the website only: the contact form, chat, map and technical data. Data processed during visits to the practice (including therapy records) is described in a separate notice you will receive at the practice.

3. Contact form

If you send a message through the form, we receive your name, a phone number or e-mail address (at least one of them) and the message itself. We use them only to reply and to arrange a visit if needed.

The legal basis is your consent, which you tick before sending (Art. 6(1)(a) GDPR), and our legitimate interest in handling enquiries (Art. 6(1)(f) GDPR).

Please do not include health details in the form — your name, contact and a short question are enough. We will discuss details by phone or during a visit.

We keep messages for: 6 months from the last contact.

4. Phone contact

If you call the number shown on the website, we use your phone number and the information you give only to talk with you and arrange a visit. Calls are not recorded through this website.

5. Website chat

The chat is provided by Tidio. The chat script loads only after you click “Start chat” — before that it is not downloaded and sends no data. Once started, the provider processes the conversation and technical device data on its own terms, under its privacy policy. The basis is your voluntary action (Art. 6(1)(a) GDPR).

6. Google Maps

The map in the contact section comes from Google. We load it only after you consent to third-party cookies. Once loaded, Google may receive your IP address and browser data and set its own cookies. Without consent the map does not load — you can use the link to open the map in a new tab.

7. Hosting and statistics

The website is hosted by Vercel Inc. The server records technical logs (IP address, time and address of the requested page, browser data) to keep the site secure and working (Art. 6(1)(f) GDPR).

We use Vercel Analytics for aggregated visit statistics. It does not use cookies and does not build user profiles.

8. Recipients of data

Data may be entrusted or disclosed to the service providers the website uses:

• Vercel Inc. — hosting and statistics,
• Resend — delivering form messages to the practice's mailbox,
• Tidio — chat (after you start it),
• Google — map (after your consent).

9. Transfers outside the EEA

Some of these providers are based in the United States or process data outside the European Economic Area. Transfers rely on GDPR mechanisms, in particular the EU–US Data Privacy Framework or standard contractual clauses — as documented by each provider.

10. Your rights

You have the right to:

• access your data and obtain a copy,
• rectify your data,
• erase your data,
• restrict processing,
• data portability,
• object to processing based on legitimate interest,
• withdraw consent at any time (without affecting the lawfulness of processing before withdrawal),
• lodge a complaint with the President of the Personal Data Protection Office (UODO), ul. Stawki 2, 00-193 Warsaw, Poland.

11. Voluntary data and profiling

Providing data in the form is voluntary but necessary for us to reply. We do not make automated decisions about you and do not use profiling.
